Tóm tắt những đóng góp mới của luận án:

Rối loạn chức năng nội mạc là hiện tượng mất cân bằng của các yếu tố nội mô bao gồm tăng kết dính bạch cầu, tăng tính thấm tế bào, tăng sinh tế bào cơ trơn mạch, oxy hóa lipoprotein. Lớp nội mạc có vai trò quan trọng trong ổn định trương lực mạch máu và tính lưu động của các thành phần trong máu. Hút thuốc lá, tình trạng lão hóa, tăng cholesterol máu, tăng huyết áp, tăng đường huyết sẽ khởi phát quá trình viêm mãn tính, làm giảm yếu tố chống đông, trong khi tăng giải phóng các chất gây co mạch. Rối loạn chức năng nội mạch gây ra hiện tượng co mạch và tái cấu trúc mạch, khởi phát quá trình viêm, gây kết dính tiểu cầu, hình thành vi huyết khối. Các hiện tượng này dẫn đến hình thành và tiến triển các mảng xơ vữa, dẫn đến các bệnh tim mạch như bệnh mạch vành, bệnh mạch chi dưới, tai biến mạch não. Do đó, để ngăn chặn các biến chứng tim mạch, việc chẩn đoán sớm, đánh giá mức độ và can thiệp tình trạng rối loạn chức năng nội mạc là cần thiết.

Trong thực hành lâm sàng, rối loạn chức năng nội mạc có thể được đánh giá thông qua độ dày lớp nội trung mạc động mạch, mức độ giãn mạch bằng siêu âm tim. Tại Việt Nam, đã có các nghiên cứu đánh giá rối loạn chức năng nội mạc ở bệnh nhân đái tháo đường týp 2, bệnh nhân tăng huyết áp, bệnh mạch vành, bệnh nhân suy thận. Nghiên cứu này của học viên là nghiên cứu đầu tiên đề cập đến tình trạng rối loạn chức năng nội mạc ở phụ nữ mãn kinh tại Việt Nam, với đánh giá tổn thương nội trung mạc động mạch đùi, giãn mạch qua trung gian dòng chảy động mạch cánh tay và các yếu tố liên quan. Kết quả của đề tài này là bằng chứng khoa học về tình trạng tổn thương nội mạc ở phụ nữ sau mãn kinh, từ đó gợi ý các biện pháp can thiệp hiệu quả để ngăn ngừa biến chứng tim mạch cho nhóm người này.

Bên cạnh đó bệnh tim mạch do vữa xơ động mạch là bệnh của thời hiện đại, đang tăng nhanh, được xem như bệnh dịch không lây. Bệnh có nhiều yếu tố nguy cơ, việc điều trị còn gặp nhiều khó khăn, vì vậy việc phát hiện sớm có ý nghĩa quan trọng trong điều trị làm chậm tiến triển bệnh. Đối tượng phụ nữ mãn kinh là đối tượng đặc biệt, có giảm nồng độ estradiol gây nhiều rối loạn chuyển hóa, đồng thời là đồi tượng tuổi có hội tụ nhiều nguy cơ tim mạch. Vì vậy, việc nghiên cứu độ dày nội trung mạc động mạch đùi và giãn mạch qua trung gian dòng chảy động mạch cánh tay ở PNMK sẽ có ý nghĩa khoa học góp phần tăng thêm bằng chứng về VXĐM giai đoạn sớm ở PNMK từ đó có hướng điều trị các nguy cơ tim mạch để hạn chế tiến triển bệnh.

Mặt khác những năm gần đây, cùng với sự phát triển nhanh chóng kinh tế, xã hội, mô hình bệnh tật tại Việt Nam đã có sự thay đổi rõ rệt: từ các bệnh truyền nhiễm sang các bệnh không lây nhiễm. Xơ vữa động mạch là nguyên nhân hang đầu gấy biến chứng tim mạch và tử vong tim mạch thường gặp nhất. Phụ nữ mãn kinh gia tăng nguy cơ bị xơ vữa động mạch. Vì vậy đề tài có giá trị thực tiễn cao.

Summary of new main scinetific contribution of the thesis:

Endothelial dysfunction is an imbalance of endothelial factors including increased leukocyte adhesion, increased cell permeability, vascular smooth muscle cell proliferation, and oxidation of lipoproteins.The endothelium plays an important role in stabilizing vascular tone and fluidity of blood components. Smoking, aging, hypercholesterolemia, hypertension, and hyperglycemia trigger chronic inflammation, reducing the anticoagulant factor, while increasing the release of vasoconstrictors. Intravascular dysfunction causes vasoconstriction and re-structure of the vessels, initiating inflammatory process, causing platelet adhesion, forming microscopic clots.These phenomena lead to the formation and progression of plaques, leading to cardiovascular diseases such as coronary artery disease, lower limb vascular disease, stroke. Therefore, in order to prevent cardiovascular complications, early diagnosis, assessment of the severity and intervention of the endothelium is essential.

In clinical practice, endothelial dysfunction can be assessed through the arterial endothelial thickness, the degree of vasodilation by echocardiography.

In Vietnam, there have been studies evaluating endothelial dysfunction in patients with type 2 diabetes, patients with hypertension, coronary artery disease, patients with kidney failure.

This study of the participants is the first to address endothelial dysfunction in menopausal women in Vietnam, with assessment of femoral endothelial injury, vasodilatation mediated by the brachial artery flow and related factors. The results of this topic are scientific evidence of endothelial damage in postmenopausal women, thereby suggesting effective interventions to prevent cardiovascular complications for this group of people.

Besides, cardiovascular disease caused by atherosclerosis is a modern, rapidly increasing disease that is considered a non-contagious disease. The disease has many risk factors, and treatment is still difficult, so early detection has important implications in treating to slow disease progression. Subjects of menopausal women are special subjects, with reduced levels of estradiol, causing many metabolic disorders, and at the same time age subjects with convergence of cardiovascular risks. Therefore, the study of femoral endothelial thickness and vasodilation mediated by the brachial artery flow in menopausal women will have scientific implications for further evidence of atherosclerosis stage Since early menopausal women tend to treat cardiovascular risks to limit disease progression.

On the other hand, in recent years, along with the rapid socio-economic development, the disease pattern in Vietnam has changed markedly: from infectious diseases to non-communicable diseases.

Atherosclerosis is the leading cause of cardiovascular complications and death among the most common.

Menopausal women have an increased risk of atherosclerosis. So the topic has high practical value.
