Tóm tắt những đóng góp mới của luận án:
* Đặc điểm lâm sàng, cận lâm sàng và mức độ nặng:
- Triệu chứng nôn, buồn nôn chiếm 36,4%; 35,2% có tăng áp lực ổ bụng.
- Số lượng bạch cầu trung bình là 10,46 ± 3,90 G/L, nồng độ hematocrit trung bình là 0,466 ± 0,115 L/L. Nồng độ triglyceride và cholesterol trung bình chung lần lượt là 27,31 ± 22,75 và 15,24 ± 10,04mmol/l.
- Theo Atlanta 1992 sửa đổi 2007 có 24,8% viêm tụy cấp mức độ nặng, 35,2% viêm tụy cấp mức độ vừa và 48% viêm tụy cấp mức độ nhẹ. Có mối tương quan thuận giữa nồng độ triglyceride máu với mức độ nặng theo các bảng điểm SOFA và Ranson.
* Hiệu quả điều trị của PEX trên BN VTC do tăng TG máu.
- Nồng độ TG và Cholesterol thay đổi sau PEX lần 1: giảm từ 31,87 mmol/l xuống 9,3mmol/l và giảm từ 18,74 mmol/l xuống 7,12 mmol/l.
- Nhóm PEX có mức triglyceride và cholesterol cao hơn và giảm nhanh hơn so với nhóm không PEX.
- Nhóm có PEX sớm có mức giảm triglyceride và cholesterol nhanh hơn so với nhóm PEX muộn.
- Sau 48 giờ, nhóm điều trị PEX có điểm SOFA và Ranson thấp hơn nhóm không điều trị PEX, p <0,05.
- Nhóm bệnh nhân vừa và nặng được PEX có số ngày nằm viện và nằm ICU thấp hơn nhóm tương tự không được PEX (tương ứng là 8,93 ±5,89 so với 14,1 ±6,31 ngày và 5,53 ±1,35 so với 7,82 ±4,23 ngày).
* Độ an toàn của phương pháp PEX.
- Tỷ lệ biến chứng tắc quả lọc 18/93 (19,4%). Tỷ lệ số lần PEX phản ứng dị ứng tại chỗ và toàn thân lần lượt là: 4/93 và 7/93. Tỷ lệ BN NK chân catheter TM đùi, hạ HA khi PEX lần lượt là 2,4% và 3,6%.

Summary of the new contributions of the thesis:
* Clinical characteristics and severity of acute pancreatitis due to hypertriglyceridemia:
- Symptoms of vomiting, nausea accounted for 36.4%, 35.2% with increased abdominal pressure.
- The average number of white blood cells is 10.46 ± 3.90 G/L, the average hematocrit is 0.466 ± 0.115 L/L.
- The average triglyceride and cholesterol levels are 27.31± 22.75 and 15.24 ±10.04 mmol/l, respectively.
- 24.8% of severe acute pancreatitis, 35.2% of moderate acute pancreatitis and 48% mild acute pancreatitis (according to Atlanta).
- There is a positive correlation between blood triglyceride levels and severity according to SOFA (r=0,33) andRansonscore (r=0,35).
* The therapeutic effect of PEX in patients with acute pancreatitis due to hypertriglyceridemia.
- The concentration of TG and Cholesterol changed after the 1st PEX: decreased from 31.87 mmol/l to 9.3mmol/l and from 18.74 mmol/l to 7.12 mmol/l, respectively.
- The PEX group has higher triglyceride and cholesterol levels and decreases faster than without PEX group. The group with early PEX had a lower level of triglycerides and cholesterol than the lately PEX group.
- On day 2nd, the group with PEX had smaller SOFA and Ranson scores than the without PEX group; the difference was statistically significant.
- Moderately and severely AP patients with PEX had a lower number of days of hospitalization and ICU than the without PEX group (8.93 ± 5.89 compared to 14.1 ± 6.31 days and 5.53 ± 1.35 vs. 7.82 ± 4.23 days).
* PEX Complications
- The rate of coagulation filter complications is 18/93 (19.4%). 11/93 times of PEX had 1st degree anaphylaxis. The ratio of patients infected at an adjacent catheter was 2.4% and had low blood pressure when PEX was 3.6%.
